
Susanna Barton, MS Gero, founder of Grand Plans and mindful aging practitioner at the Center for Wellbeing in Jacksonville FL, is partnering with The Home Team, powered by APlan2Age, a nationwide initiative bringing together professionals to help older adults and their families plan for aging, find resources and build stronger support systems.
The partnership aligns with Grand Plans’ focus on preventative gerontology, which encourages individuals and families to prepare for later life through education, practical planning and candid conversation. Through her work, Barton helps people consider decisions about their future, organize essential information and communicate their wishes before an urgent situation requires action.
The Home Team connects professionals across aging services, healthcare, financial planning, legal services, housing, home care and other supportive industries. By encouraging collaboration across these fields, the initiative aims to make resources more accessible to families whose questions about aging often span several areas of expertise.

A distinctive component of the initiative is Senior Spaces TV, an interactive, HGTV-style experience designed to help the public visualize what planning for aging can look like in everyday life.
