Grand Plans Launches Free Online Resource to Help Northeast Florida Families Navigate Aging

Finding help for an aging parent, a spouse or yourself often begins with questions that are difficult to answer: Who can help someone stay safely at home? Where can a family turn after a dementia diagnosis? Who handles transportation, daily bills or the logistics of downsizing?

Grand Plans has launched Grand Plans Connect, a free online resource designed to help Northeast Florida residents find their next step. Available at grandplansconnect.com, the website brings together 235 listings across 30 categories, with a focus on Jacksonville and Duval, St. Johns and Clay counties.

Created by Susanna Barton, MS Gero, founder of Grand Plans and author of Grand Plans 2.0: How to Mitigate GeriDrama in 20 Easy Steps, the platform extends her work in Mindful Aging and preventative gerontology by making local resources easier to find and use.

“People often discover how much they need to know about aging when they are already facing a difficult decision,” Barton said. “Grand Plans Connect gives them a place to begin, whether they are responding to an immediate need or taking steps now to prepare for the years ahead.”

Visitors can browse the directory, search by a provider’s name or service, and filter listings by category and county. A guided “Find help” feature asks a few questions about whom they are helping, where that person lives and what support they need, then identifies relevant directory listings. No account is required to explore the resources.

The directory addresses the range of decisions and practical needs that can accompany aging, including:

  • Care management, professional caregiving and caregiver support.
  • Dementia assessment, treatment and community resources.
  • Occupational therapy, home modifications and medical equipment.
  • Senior living, housing specialists, moving and organizing.
  • Transportation, meal services and everyday assistance.
  • Daily money management, financial planning and public benefits.
  • Elder law, estate planning and protection against exploitation.
  • Hospice, palliative care and advance planning.
  • Social connection, learning, volunteering and senior centers.

Listings include nonprofit organizations, public programs and private businesses, with phone numbers, websites and published email addresses where available. Visitors contact providers directly.

Grand Plans Connect launches as a Northeast Florida beta, with an invitation for aging-services professionals in the three counties to submit their organizations for consideration through the website. Submissions are reviewed before publication.

The directory’s published contact information has been reviewed online. Families should confirm service areas, eligibility, costs, availability and relevant professional credentials directly with providers.

The launch builds on Grand Plans’ broader mission to help people prepare for aging through education, practical planning and family conversations, bringing those conversations closer to the organizations and professionals who can help.
